Re: Selling your Adventist books to Lost-N-Found Books.

I am shipping ____ boxes of used books to you. All are by Adventist authors, publishers, or are about Adventists. Each box is insured for $50.00 per box.

I have enclosed a copy of my name, address, and phone number in each box. I have also written on the outside of each box the box number (i.e., Box 1 of 3, Box 2 of 3, etc.)

If you agree to sell us at least 20 lbs or 20 Adventist books, you can request prepaid US Postal shipping labels for your convenience and savings. These labels are only good for shipments within the U.S. and territories; all other countries will be reimbursed only for U.S. Media Mail (book rate) shipping. I understand that if I have not enclosed at least 20 lbs or 20 Adventist books, I will be reimbursed for only 1/2 of the media mail postage.

A copy of this form MUST be included with each box in the shipment! Books shipped without this Payment Request Form will be assumed as a donation to Lost-N-Found's missionary work.

I understand that the appraised value of each book will depend on availability, author, age, appearance, and whether it is autographed. I understand I will receive (whichever is greater) up to 30% of the retail value OR $5.00/lb for books printed in or before 1915 and, for items after 1915, $0.75/lb for hardcover—$0.25/lb for all other acceptable items.

Adventist books with no commercial value, but autographed or associated with a prominent Adventist individual, may be accepted. Please call if you have more than 10 copies of a single title. If you send more than 10 copies of a single title without prior approval, we will pay only for the first 10 copies.

(NOTE: Shipping on non-Adventist books, or books that have no resale value, will not be refunded.)
